Operation Welcome Home Case Manager: Outpatient Program
We currently have an opportunity for a full-time, non-exempt Case Manager for our Operation Welcome Home Outpatient Program. The OWH Case Manager for Outpatient will plan and facilitate outreach strategic efforts, including multimedia presentations, to Iraq and Afghanistan veterans and their families. The Case Manager will work in collaboration with Outreach liaisons to provide essential education about PTSD, TBI and other vital issues affecting the current veteran population while attracting veterans to the Operation Welcome Home program for services. We are looking for an energetic and outgoing individual to be a part of our team. Responsibilities include the following:
Job Duties:
- Assess new veterans during screening and intake.
- Keep client files current through a computerized tracking system and maintain ongoing files (handwritten and on computer)
- Develop individualized treatment plans, review records and evaluate progress in achieving goals, providing ongoing support to the veterans.
- Coordinate medical, legal, dental and/or psychological services as needed.
- Work consultatively with other staff members to ensure appropriate services are rendered. Maintain high level of customer service to the population.
- Maintain referral lists; be aware of local agencies and of services within surrounding communities.
- Facilitate groups as assigned, including on weekends or evenings as needed.
- Participate in conferences, workshops, special projects, and staff meetings.
- Partner with New Directions' employment team and devise strategies to get veterans into the workforce.
- Effectively advocate for the veteran.
- Assist in helping the veteran successfully transition into community housing, and provide follow-up services.
- Work collaboratively with other staff members as part of a team.
- Oversee daily activities of the house.
- Facilitate aftercare and alumni activities.
- Other duties as assigned.
Job Requirements:
- Bachelor's Degree in social service or related field. Appropriate related experience can be substituted
- Recently honorably discharged OEF/OIF veteran highly desired.
- Working knowledge of community resources and services. Ability to learn and create additional resources.
- Experience in working with a multidisciplinary treatment team.
- Excellent interpersonal and oral and written communication skills, patience, diplomacy and a high level of customer service.
- Word processing skills required.
- Willingness to work flexible hours.
- Ability to drive and be covered by New Directions' insurance company.
- Minimum 1 year clean and sober, if in recovery.
- Minimum of three (3) years of work experience is preferred.
